Here is a question that has always been a bit controversial, I have always been of the opinion that Jason Voorhees was indeed mentally deranged as a result of his deformed condition.
Tom Savini, the creator of the Jason character (literally), stated that he got the idea for Jason being deformed from this homeless man who lived in the town in which he grew up (somewhere in NJ). Savini described Jason as being hydrocephalic.
[Hydrocephalus is a medical condition in which there is an abnormal accumulation of cerebrospinal fluid in the ventricles, or cavities, of the brain. This may cause increased intracranial pressure inside the skull and progressive enlargement of the head, convulsion, tunnel vision, and mental disability.]
This is supported later on in the series where Jason mistakes Ginny for his mother, and the fact that he keeps his mom's severed head on an alter.
Jason Voorhees is hardly a picture of mental health.
